Prices may vary based on the test for those without insurance. Doctor's Orders Accepted Any lab orders prescribed by your doctor can be collected in the comfort of your home or office. NOTE: I can provide requisition forms if needed. To process your test, please provide the following information on doctors orders: 1. Patient Information: - Name, date of birth, and other demographic details. - Insurance information (a copy of the insurance card is sufficient). 2. Healthcare Provider Information: - Name, address, and contact details of the provider ordering the tests as well as NPI. 3. Reason for the Blood Test: - The healthcare provider should indicate the reason for the test, including symptoms or conditions being screened (diagnostic codes). 4. Type of Blood Test: - Specify the exact test being ordered, including any special instructions for sample collection. 5. Date of Request: - Record the date the requisition was completed by the healthcare provider. 6. Signature: - The requisition must be signed by the healthcare provider to confirm the necessity of the requested tests. Having a complete and accurate requisition ensures that the correct tests are performed and are covered by insurance, with that being said results are accurately interpreted to support proper diagnosis and treatment.
