Service Description

A trained phlebotomist will enter the building early AM to collect blood samples from patients. This includes venipuncture (drawing blood from a vein) in-office testing will be pending until further Notice ANY SPECIMENS COLLECTED BY THE NURSE WILL BE LABELED AND STORED PROPERLY please refer to the directory for detailed information about labeling and storing. * The phlebotomist provides specimen collection containers such as specimen cups, culture swabs, viral swabs, etc. Viral swabs will be provided and collected at the time of the phlebotomist's arrival for accurate analysis due to being time-sensitive Schedule Facility will be serviced early Am, no later than 12:00 Am The office will be open at 8:00 AM closing time 3:00 pm Stat labs can be ordered from 04:00 AM - 08:00 PM Monday - Wednesday until further notice, with Additional charges to the facility or the resident's card on file. Payment Terms Servicing fees are a flat rate of $40.00 per resident and will be charged to the COF (Card on File), there will be additional fees for any Stat orders ( STAT fee is an additional $20.00 ) For Whatever Reason the charge does not go through the resident's COF, The facility will be billed and should be taken care of promptly by check or card, Cash is not recommended. (Labs ordered on a resident with a balance will not be collected until the balance is paid off) *Any specimen pick-up orders are a flat rate of $25.00 Ex: urine/ fecal matter/ swabs. *Weekly rate option charged to the Facility* NOTE: This is just an option for servicing, not a charge. Servicing package WEEKLY RATE a flat fee of $500.00 per week not exceeding 15 patients draw per week *urine/fecal pick up is included in the set weekly rate (additional fees may apply, Ex: each STAT will be an additional $20.00, culture swabs/viral swabs are an additional $20.00) STATS AND SWABS WILL BE BILLED SEPARATELY ON FOLLOWING MONDAY EACH WEEK AND NEED TO BE TAKEN CARE OF PROMPTLY. Servicing package WEEKLY RATE a flat fee of $750.00 a week not exceeding 20 patients draws per week *urine/fecal pick up are included in the set weekly rate.
