Service Description

This Contract provides comprehensive blood collection services both on-site at the clinic. Services include venipuncture and capillary collection, urine collection, stool samples, proper labeling, handling, and transportation of specimens to ensure integrity. Following strict infection control protocols. I will collaborate with staff to coordinate patient care, and support the clinic in maintaining quality and compliance standards for phlebotomy services. Schedule Servicing times below 6:00 AM closing time 3:00 pm Mondays - Thursdays 6:00 AM to 11:00am Fridays Stat labs can be ordered from 06:00 AM - 08:00 PM 7 days a week, with Additional charges of $15.00.( STAT LABS RECOMMENDED FOR INSURED PATIENTS TO AVOID LABORATORY BILL) To order stat labs please call 417-676-2734 or 417-593-1881. If the line is busy, Leave a voice mail or MSG. Payment Terms Servicing fees are a flat rate of $35.00 per patient that is insured and can be charged to the patient card or cash is accepted, there will be additional fees for any Stat orders ( STAT fee is an additional $15.00 ) Servicing fees for self-pay patients are $70.00 including the testing fee. ( STATS ARE NOT ELIGIBLE FOR SELF-PAY ) Note: If the patient would like to go to the office for services $50.00 is the set rate.
